You have not been to our practice before and are going to register as a new patient at Botman Tandartsen. We ask you to fill out a registration form and send it to us via a secure environment. We will create a file for you and contact you within two working days for an appointment. You will also receive an invitation by email to create an account. Via this account you can, if you wish, schedule the appointment yourself via the online appointment module.
You will also receive an invitation from MijnMedicijn.nl to securely complete a health questionnaire. It is important that we are always informed of changes in your medication use and health. Therefore, it is helpful if you have an account on Mijn Medicijn. You then only need to report the changes and do not have to fill out a new questionnaire every time.
